I/we hereby apply to use the premises and facilities of HOPE Centre, Midland for the dates and for the purposes set out in our application. I/we acknowledge, confirm, and agree that: I/we have read HOPE’s facility use policy and the regulations regarding use of the premises; my/our application will be evaluated in accordance with the facility use policy, and it is a condition of my/our application being reviewed that I/we have agreed that the review of the application will be based on and governed by the policy; and if approved, I/we will be required to sign a facility use agreement, including price and additional terms.
